Download "Graduation Request Form"  

2. บันทึกข้อมูลออนไลน์ "แบบรายงานผลการศึกษา" หลังจากนั้นให้พิมพ์ข้อมูลเฉพาะหน้าแรกที่มีชื่อ-สกุลของตนเอง (จากอีเมล)
